Program, transfer an existing Independent 529 Plan account to another
beneficiary, or transfer from a U.S. Savings Bond or Coverdell Education
Savings Account.

Use this form to redeem prepaid tuition certificates at an Independent 529 Plan member school.

1. Withdrawals for your beneficiary's qualified higher education

death, disability or scholarship.

Complete this form, in addition to the Refund Withdrawal form, if
